Mobile MedMassage PLLC brings restorative wellness to where people already are. This policy explains what information we collect when you contact us or book with us, why we collect it, who we share it with, and the choices you have. We have written it in plain language on purpose.

Who we are

Mobile MedMassage PLLC ("we," "us," or "our") is a mobile restorative wellness company serving Scottsdale, Cave Creek, and Carefree, Arizona. This policy applies to mobilemedmassage.net, to our contact and intake forms, to our text and email communications, and to information we collect in the course of providing our services.

Mobile MedMassage PLLC designs and delivers restorative wellness experiences supporting relaxation, mobility, and well-being. We do not diagnose, prescribe, or provide medical treatment. We are not a health plan, a health care clearinghouse, or a health care provider that transmits health information in connection with covered transactions, and so we are not a covered entity under HIPAA. Information you share with us about your comfort, mobility, or health history is not protected health information under that law. We treat it as confidential regardless, and we handle it as described below.

Information we collect

Information you give us directly

• Contact form. First name, last name, email address, phone number, the type of service you are interested in, and whatever you write in the message field. Email and phone are required; the rest is optional.
• Discovery calls and correspondence. What you tell us by phone, text, or email about who the session is for, what you are hoping for, and any access or mobility considerations we should plan around.
• Intake forms. Before a Mobile Wellness Suite session, we send an intake form by text. At a Healthy Hour™ event, guests complete a brief intake and release by QR code at check-in. These ask for comfort and mobility information, areas to focus on or avoid, and anything that would make a session unsafe or uncomfortable for you. You decide how much to share; sharing less may limit what we can safely do.
• Consents and acknowledgements. Your responses to our liability release, health check-in, photo and video, and marketing contact acknowledgements, and the date and time you gave them.
• Host and partner information. For organizations booking a Healthy Hour™ or a Professional Partnership: contact names, titles, organization details, venue details, and expected guest counts.
• Payment information. Payments are processed by third-party payment processors. We receive confirmation of payment and limited transaction details. We do not store full card numbers.

Information collected automatically

• Usage and device data. When you visit our website, our platform and analytics providers may record your IP address, browser and device type, pages viewed, referring page, and the dates and times of your visits.
• Cookies and similar technologies. Our website and forms use cookies to keep the site working correctly, remember form progress, and understand how the site is used. Most browsers let you refuse or delete cookies; some parts of our forms may not work properly if you do.

How we use your information

• To respond to your inquiry, personally, usually within one business day.
• To schedule, prepare for, and deliver your session, including sending your intake, reminding you of your appointment, letting you know when we have arrived, and checking in two days afterward.
• To assign the right therapist and plan around what makes movement easier or harder for you.
• To provide hosts and partners with an event recap: guests served, sessions delivered, and participation compared to expected headcount. Recaps report numbers, not individuals, and never include what any guest told a therapist.
• To process payments and keep business records.
• To send marketing and promotional messages, but only if you have separately consented to receive them.
• To improve our services and our website.
• To meet our legal, insurance, tax, and professional-licensing obligations, and to establish or defend legal claims.

Text messages

Our contact form carries two separate consent checkboxes, and they do different things.

• Non-marketing texts cover your inquiry, appointment scheduling, intake, reminders, arrival notification, and follow-up. These are the messages that make a session work.
• Marketing texts cover offers, promotions, and service news. This consent is entirely optional and is never a condition of booking anything.

Message frequency varies. Message and data rates may apply. Reply HELP for assistance or STOP to opt out at any time; STOP applies to the category you opted out of, and opting out of marketing texts does not stop appointment messages you have asked for. Consent to receive marketing messages is not a condition of purchase.

Mobile information collected for the purpose of SMS consent is not sold, rented, or shared with third parties or affiliates for their own marketing purposes. It is shared only with the messaging providers who deliver the messages on our behalf.

Who we share information with

We do not sell your personal information, and we do not share it with third parties for their own marketing. We share it only in these situations:

• Service providers. Companies that run things on our behalf and are permitted to use the information only to do that work: our customer relationship and website platform, text and email delivery providers, scheduling and form tools, payment processors, and analytics providers.
• Your therapist. The licensed therapist assigned to your session receives what they need to serve you safely and well.
• Hosts and partners. Aggregate participation numbers only, as described above.
• Legal and safety. When required by law, subpoena, or regulator, or where we believe in good faith that disclosure is necessary to protect the rights, property, or safety of any person.
• Business transfer. If Mobile MedMassage PLLC is involved in a merger, acquisition, or sale of assets, your information may transfer as part of that transaction. We will tell you before your information becomes subject to a different privacy policy.

Photography and video

We photograph or film at an event only when it has been arranged with the host in advance. Consent is captured at check-in, and guests who decline are not photographed. If you appear in a photograph or video and later want it removed from our materials, email [email protected] and we will remove it from anything we control.

How long we keep information

We keep contact and inquiry information for as long as needed to serve you and for a reasonable period afterward. We keep intake forms, releases, and consent records for as long as our insurance, professional, and legal obligations require. We keep financial records for the periods tax law requires. When information is no longer needed for any of these purposes, we delete it or render it unreadable.

How we protect information

We use administrative, technical, and physical safeguards appropriate to the sensitivity of the information, including access controls that limit intake information to the people who need it. No method of transmission or storage is completely secure, and we cannot guarantee absolute security. Please do not send sensitive information such as government identification numbers or full payment card numbers by text or email.

Your choices and rights

• Access and correction. Ask us what we hold about you, and ask us to correct it if it is wrong.
• Deletion. Ask us to delete your information, subject to records we are required to keep.
• Opt out of marketing. Reply STOP to a marketing text, use the unsubscribe link in an email, or simply tell us.
• Withdraw a consent. Withdraw a photo, video, or marketing consent at any time going forward.

Depending on where you live, you may have additional rights under your state's privacy law, including the right to know what personal information we collect and how we use it, the right to request deletion or correction, the right to opt out of the sale or sharing of personal information and of targeted advertising, and the right not to be discriminated against for exercising any of these rights. We do not sell personal information or use it for targeted advertising. To make any request, contact us using the details below. We will verify your identity before acting, and we will respond within the time your law allows. If we decline a request, we will explain why and how to appeal.

Children

Our website and our marketing are directed to adults. We do not knowingly collect personal information online from children under 13. Guests under 18 are served only with the consent of a parent or legal guardian, who provides the intake and consents on the minor's behalf and remains present. If you believe a child has given us information online, contact us and we will delete it.

Third-party links

Our website may link to sites we do not operate. We are not responsible for their content or their privacy practices, and this policy does not apply to them.

Changes to this policy

We may update this policy as our services or the law change. The effective date at the top will always show the current version. If we make a material change to how we use information you have already given us, we will tell you directly.
